Kasabach-Merritt syndrome (KMS) is a rare hemangioma developing consumptive coagulopathy with large vascular lesions. We present the case of a 27-year-old male who, preexisting KMS in his right lower extremity, had left compressive femoral neuropathy due to huge intramuscular iliopsoas hematoma. Most adult patients with KMS have been taken care of hemorrhagic condition, infection, shock of hemangioma. However, according to varying conditions they sometimes need to be cared with internal hemorrhage in spite of no trauma history. Before the hemorrhage develops to hematoma, evaluation must be done enough tomake proper diagnosis. Hematoma can suppress any tissues or organs around it. We believe this report of femoral nerve compression is the first report of neuropathy in KMS patients. (Clinical Pain 2017;16:36-39)
