In recent years, an increasing number of patients have complaint related to hip pain. It is assumed that active physical activity, damage from participating sports activity, increasing number of elderly population mainly contribute to increase degenerative hip joint disease. It is important for clinicians to make a differential diagnosis whether the pain origin from extraarticular lesion or intraarticular lesion. Typical intraarticular lesions are femoroacetabular impingement syndrome, adhesive capsulitis, osteoarthritis, avscular necrosis and hip osteoporotic fractures. Individualized approaches are required because each of disease appears according to age and clinical progress varies by disease. Therefore, we reviewed the characteristics of each disease, physical examination, screening methods and differential methods for other diseases. And we introduced the principle of conservative management and practical key points. (Clinical Pain 2016;15:13-19)
