The spasticity of upper extremities is one of the most cum-bersome problems in many central nervous system disorders. Injection of the botulinum toxin into affected mus-cles has been proven safe and effective in reducing spasticity. Accurate localization is especially importance to maximize its efficacy because the toxin does not cross the physical barriers such as the fascia. Recently comparable outcomes were reported between ultrasound-guided and stimulator-guided injection techniques in reducing spasticity after stroke. The present review focuses on the techniques of ultrasound-guided botulinum toxin injection into the major upper extremity muscles. (Clinical Pain 2014;13:9-15)
