Baclofen can induce significant neurologic side effects. We report a case of acute Baclofen induced encephalopathy in a 44-year old female patient. She was diagnosed with chronic kidney disease due to focal segmental glomer-ulonephritis and has been received peritoneal dialysis for 1 year. She had a acute basal ganglia hemorrhage in the right cerebral hemisphere and then received surgical management. After 1 months after the onset of hemor-rhage, she has developed pain, swelling and paresthesia of the left upper extremity. She was diagnosed with com-plex regional pain syndrome. Medications were limited be-cause of the possibility of renal intoxication. She was pre-scribed Baclofen 5 mg, additionally. After then she pro-gressively showed confused mental status and dysarthria and was diagnosed with acute metabolic encephalopathy. We started intensive hemodialysis and gradually she showed improvement in mental status and dysarthria. After 6 day treatment, she had fully recovery. (Clinical Pain 2013;12:95-98)
